Insurance Validation

Your plan may require you to provide documentation indicating that you carry certain levels of insurance. Let’s take a look at what is required so we can get you going quickly.

Either the official “Declaration” from your insurance company or an “Acord Certificate” available from all insurance companies may be used as proof of insurance (an insurance card is not valid proof of insurance). The “Declaration” or “Acord Certificate” must clearly display:

  • Policy Number
  • Minimum coverage required by the plan
  • Expiration Date

Note: When uploading your insurance documentation, it is best to upload as a PDF file since the Declaration is usually multiple pages. Most insurance companies provide a PDF download of the Declaration from their website. If you have difficulty, you can always fax your documentation to 888-512-1663.

Important: The file name must not contain any special characters (!@#$%^&*()_+=-?/,<>’;:”). We recommend using something simple such as “Bob Smith Insurance”.
